Real Life Choices and Self Directed Supports
The Arc Gloucester is a qualified provider through the New Jersey Division of Developmental Disabilities (DDD) for both Real Life Choices and Self-Directed Supports budgets.  Qualified services include:   Camp Sun ‘N Fun (day and residential camping),  in-home supports, day programs, supported employment, individual supports, and Life Long Learning classes for adults.  
‘Real Life Choices' (RLC) and ‘Self-Directed Supports' (SDS) are both self-directed approaches that represents a system's change for DDD in the way in which services are provided to individuals with intellectual and related developmental disabilities.  The system is changing from focusing on goals identified by professionals to outcomes developed by individuals and their families.  The goal of both RLC and SDS is to assist individuals and families to create outcomes through a Person-Centered Thinking model,Essential Lifestyle Planning (ELP).  
This tool helps individuals, and their families, go beyond the identification of available services and allows them to focus on what is important to them and how to build those choices into their Plan of Care.  RLC and SDS participants purchase services and supports based on an individual budget. The individual and family purchase services and supports from a qualified provider to achieve the outcomes they have identified in their Plan of Care.
Self-directed support services allow individuals to help identify their own activities; allow individuals to schedule their own activities; require some involvement by family and/or friends; include assistance in researching, planning and making connections from a Support Coordinator; begin with an assessment completed by phone; assign a budget that individuals can use to obtain services.  Self-directed supports focus on four areas:
  • Habilitation (up to 25 hrs. a week).  Services may include:  day program, classes or instruction (group or private), gym memberships
  • Individual Supports (hiring of Direct Support Professionals): hire someone who will be paid through the fiscal agent, or contract with a qualified Provider agency to provide staff.
  • Transportation: purchase a ride to a place that is paid through the budget.
  • Supported employment:  job coaching can be purchased once you have used Division of Vocational Rehabilitation Services (DVRS) and Commission for the Blind and Visually Impaired (CBVI).
